Returning to Hockey after Illness:
If your son / daughter has missed hockey due to being sick, the following form must be filled out, it will be sent automatically to the [email protected] email. You will receive a copy, please forward this to your teams coach and trainer. They are not able to take part in any PHA activities until this is completed and the Health team clears them for a return to play. To be cleared by the health team they must have a negative Covid-19 test or be symptom free for 48 hours and pass our screening questionnaire.

PHA Protocol
The PHA followed OHF and City of Peterborough Health regulations to create a return to play policy. This was handed and accepted by the OMHA and City officials and will be followed by all involved.

Coaches / Trainers / Ref / Timekeeper / Player / Spectator Tracking form:
Every person entering a facility must be tracked for contact tracing purposes.  There must be no more then 50 in an arena at a time and home teams will log the refs and timekeepers. All forms are handed in monthly to the VP of the division.
